We have never seen this issue occur except when participants have completed the survey in Safari. If you're willing, perhaps you could assist us in diagnosing the issue, so that we can resolve it more quickly.

If you don't mind, here are a few questions that might help us figure out what happened:

1) Was the survey opened in a new window, or a new tab?
2) What type of window or tab were you using to complete the survey -- private/incognito, or public?
3) Did you use https or http?4) Did you use a Mac or PC?
4) What operating system did you have on your computer at the time?
5) Did you exit the window/tab and return to it at any point?
6) Did you use any special characters while typing, e.g. accents on letters, emojis, etc.?
7) Were any questions left blank or unanswered?
8) Did you see any error messages on the screen at any point -- and if so, do you recall what they said?
9) Did any part of the study not work for you (i.e. did you have to skip any pages, did anything freeze or fail to load, etc.)? If so, which part, and was there any message about it on your screen?
9) Do you have Adobe Flash enabled?
10) Do you have cookies enabled?
11) Does your browser automatically unzip files immediately after downloading them?

Any other information you might have would also be useful, to give us any indication of what may have gone wrong.
