It wasn't a concern for the most part because the method of rending stuff had hard limits that would basically allow you to always keep the target output. That is why sprites flickered ect. Oh and people imported consoles and TVs so that they could have 60hz instead of PAL's 50hz fairly often so it obviously was a known thing.

The high refresh rate CRTs giving you a headaches is the opposite of how it worked. The cheap low refresh rates when you wanted a higher resolution is when you started to have headaches and not the other way around.
