All, I am resubmitting the design to correct for some of the confusion.

QA/items addressed

1. If there is no phone number and no address match, this is not a conclusive match and the NPI lookup will need to be performed.
   1. If the correct version of the first and last name is search nationally, and there is one result then the answer is yes to the final question.
   2. If there is more than one result nationally, then the listed state filters should be applied, if filtering by any of the listed states results in a single result, then the answer is yes. Otherwise the answer is no.
2. First and Last name were switched, this is corrected in the update hits once they republish
3. If the match isn’t conclusive before doing the search, you answer for the “over all match“ question should remain no.

In this Hit, there is no address and phone number but which NPI address and phone number are present, in these scenario when i gone for NPI lookup which ask me the NPI number, so when i considered Ref 10 digit number as NPI number still, there is no search result found, in this case could i Mark "No" for Last Questions. Please advice.

A. NO. only use First Name, Last Name and state. Phone number is not an npi number.

The interface is somewhat aesthetically pleasing, at least its a slight change from the normal HIT frame.
Pay is good, despite inefficient workflow & quite laggy/buggy program.
Cons: Robot often "hangs" for a bit between questions or takes some time to process info/answers.
Interface could be vastly improve.
Code is a pain in the tail end to copy/paste.
