    I'm really out of practice when it comes to surveys. I do them about half the speed of everyone else. Not even sure why I'm doing them.

    I wrote in on the optional note at the end. So I just quoted that and wrote the requestor. Hopefully that will be fine. I used my worker ID as code.

    Yeah, I did the same. I hate it when there is a code box, but no code given for the survey. The lack of diligence on a requestor's part is annoying.
