I sent him a message letting him know my answer choices and that the survey was borked. -_-
Sent him another one this morning so hopefully he responds or reads it.

 

I emailed him (gave him all the answers in the email!) and never heard back from him. I decided to just return it. How maddening! I know you emailed him yesterday and also gave him the answers. Looks like he is getting work for free.
