Google for "Institutional Review Board" for MIT .... or .... go to the MIT website and seach for the IRB there .... you have the tools to do it :)
If you do not hear back from that requestor, try writing to the IRB at MIT. It appears the survey is run from 'Online Survey Software I Qualtrics...
