I've been mostly MIA for the last week or so, I'm not clear on what's going on (with this or anything else, like I gotta figure out what TV is). But I had to re-verify my Amazon Payments account maybe 2 years ago even though I've had my MTurk account for years and have used it on and on since creating it. The request was out of nowhere, but it was no big deal, I sent them the info and that was that. I've just checked through my old emails and at that time (July 2015) the fax number was a toll free number - 800cc887cc0540. They specified in the contact email that the email associated with my Amazon Payments account should accompany my documentation to expedite things. HTH. Good luck.
